Memorial Gifts & Bequests
If you have been inspired by an exceptional individual or suffered the loss of a family member or friend, you may wish to recognize this special relationship through a gift to Franklin Community Health Network or one of its affiliates made in honor or in memory of that person.
Unrestricted memorial gifts and bequests become part of the FCHN Endowment. If you would like your gift restricted to a specific area or program, please let us know by filling in the appropriate section on the giving form. When making a memorial gift, please include the following information along with your donation:
- Name of the deceased and date of death
- A copy of the obituary, if available
- Name of surviving family member, benefactor, or estate executor to whom notice of the gift should be sent
- Name, address, and telephone number of the principal donor
- Amount of each donation included in gift total
The FCHN Foundation Office sends an acknowledgement to the surviving family with the name and address of the donor, but not the amount of the gift. Please also see information about named endowed funds
You may also want to consider making a bequest to FCHN or one of its affiliates in your will or naming one of them as the beneficiary of your life insurance or retirement plan. Through your will you may leave a specific monetary gift or asset, a percentage of your estate, or the residual after gifts to family or friends.
